精子凍結（社会的適応）と保管

ライフスタイルの多様化に伴い、主に独身の男性が、将来子供を持つ選択肢の一つとして、精子を凍結保存しておくためのご予約をLINEにて承ります。

当院で精子凍結を行い、グレイスバンクの精子凍結保管サービスをご利用いただきます。

事前にグレイスバンクへ会員登録と保管申込、感染症検査が必要です。

 

【ご注意】ご夫婦で不妊治療を行う目的で精子凍結をご希望の方は、初診でご来院いただいた後、受付またはお電話にてご予約となります。

 

Appointments via the LINE app are available primarily for single men who wish to freeze their sperm for future family planning.

Prior registration with Grace Bank and infectious disease testing are required.

【Please Note】 Sperm freezing for the purpose of fertility treatment as a couple is not available for online booking. After your initial consultation, please make an appointment for sperm freezing at the reception desk or by phone.

別途、グレイスバンクへ初期費用/保管料、感染症検査料のご負担がございます。

Grace Bank storage fees and infectious disease testing fees are paid separately to Grace Bank and the testing provider, respectively.

 

ご来院時の持ち物

全てお忘れなくご持参ください。お忘れの場合、精子凍結はできません。

①マイナンバーカードまたは資格確認書

②バーコードシール15枚（グレイスバンクから送付）
③精子の採取・凍結・保管に関する同意書（グレイスバンクから送付）
④1年以内の感染症検査結果（紙に印刷）

※当院は尿検査に対応しておりません。検査キット（グレイスバンクのWebサイトより検査会社へお申し込み可能）または他の医療機関をご利用いただき、検査結果は必ず紙に印刷したものをご持参ください。陽性の方の精子はグレイスバンクに保管できません。

【精子凍結を行うにあたり必要な感染症検査項目】

尿検査	淋菌（PCR）、クラミジア（PCR）
血液検査	HIV（抗原・抗体）、梅毒（TPHA）、B型肝炎（HBs抗原）、C型肝炎（HCV抗体）

Please bring all three of the following items
①My Number Card(Individual Number Card) or Health Insurance Eligibility Certificate

②15 barcode stickers (sent to you by Grace Bank)
③ Consent Form for Sperm Collection, Cryopreservation, and Storage (sent to you by Grace Bank)
④Infectious disease test results from within the past year（printed copy）

Please note: Our clinic does not provide the required urine tests. Please use a testing kit and bring a printed paper copy of your test results. Sperm from patients with positive infectious disease test results cannot be stored by Grace Bank.

【Required Infectious Disease Tests for Sperm Freezing】

Urine	Gonorrhea（PCR）、Chlamydia（PCR）
Blood	HIV（Ag/Ab）、Syphilis（TPHA）、Hepatitis B（HBsAg）、Hepatitis C（HCV Antibody）

 

当日の流れ

当日は精液検査と精子凍結を実施致します。

★2-3日程度禁欲の上でご来院ください。

★所要時間の目安は２～３時間です。状況によりお待たせすることもございますため、お時間に余裕を持ってご来院をお願い致します。

①受付	患者登録および持参書類の確認のため、約30分お時間をいただきます。この間の外出はご遠慮ください。
②採精	ご提出後、約1時間お時間をいただきます。この間は外出していただいて構いません。
③説明	結果説明とご希望の凍結本数の確認を行います。
④会計	費用のお支払いをもって終了となります。

Please abstain from ejaculation for 2–3 days before your visit.

Semen analysis and sperm freezing will be performed on the day and will take approximately 2–3 hours.

①Check-in	Please allow approximately 30 minutes at the clinic for patient registration and document verification.
②Collection	After submitting your sample, please allow approximately one hour for processing. You may leave the clinic during this time.
③Consultation	We will explain the results and confirm the number of vials you wish to freeze.
④Payment	After payment of the fees, your visit is complete.
