In accordance with the insurance coverage of infertility treatment, 【Renewal of frozen embryo】 is covered by insurance.

There are some requirements for insurance coverage, and those who are not covered will have to pay for the renewal at their own cost.

Please answer the questions when you make an appointment online to confirm whether your renewal will be covered by insurance or at your own cost.

 

If it is difficult to come to the clinic, you can also renew by wire transfer and mailing the consent form (additional handling fees will be charged).

Please read the instructions carefully, fill out the consent form, and sign in your own handwriting for each couple.

It is preferable to come to the clinic and submit the consent form when renewing your freezing procedure.

*If you wish to renew sperm freezing for special cases (only for eligible patients) or oocyte freezing for medical indications (patients who require chemotherapy or radiotherapy for treatment of the underlying disease), a consent form will be provided at the time of renewal explanation.

 

If you wish to dispose of yours, please submit a “ Agreement form for Disposal”.

However, if there is no renewal request from you after the expiration of the storage period, we will dispose of yours without “ Agreement form for Disposal”.

If you are unable to visit to the clinic and wish to renew your frozen renewal by wire transfer and mail

*Renewal by wire transfer and mail is at your own cost.

*Please note that an additional fee of ￥2,200 will be added to the renewal fee for wire transfer and mail processing.

If you are paying from overseas, the renewal fee will be paid via the PayPal system, and a PayPal system fee will be charged in addition to the renewal fee and the wire transfer/mailing processing fee.

If you wish to renew both embryos and sperm at the same time, please be sure to indicate so.

(3) Address to which we should send a copy of your consent form, receipt and statement:

*If you choose to receive the receipt by e-mail, the original receipt will be given to you at your next visit.
